What does FESS aided by a navigation system means?

FESS involves inserting a slender endoscope and tools through one’s nostrils to expand sinus passages by removing small bones and cartilages. To reach the origin of the problem and eliminate it, an ENT doctor removes layers of cartilages and tissues. These cartilages are near to vital anatomical structures such as optic nerves, anterior ethmoidal, carotid arteries and nasolacrimal ducts. Unexpected damages to these structures can cause major complications like CSF leaks, blindness, oculomotor deficits and perioperative hemorrhage.

FESS aided by a navigation system are reported to lessen intraoperative time, better the surgical outcome and reduce the workload. This system allows real time surgical navigation using preoperative computed tomographic images.

What are the complications related to FESS without the aid of a navigation system?

  • Orbital hematoma
  • Vision changes
  • Cerebrospinal fluid leak (CSF leak)
  • Intracranial damage
  • Anosmia (loss of the sense of smell)
  • Hyposmia (decreased sense of smell)
  • Periorbital ecchymosis
  • Orbital emphysema
  • Epistaxis

To make FESS safer, ENT doctors use a three dimensional image guided navigation system (IGNS) that records one’s CT scan and monitors the position of the tools inside him/her.

How is FESS aided by a navigation system useful?

It can be used for procedures like:

  • Revision sinus surgery
  • Distorted sinus structure (due to development, postoperative or trauma)
  • Sino-nasal polyposis
  • Pathology involving the frontal, posterior ethmoid and sphenoid sinuses
  • Diseases beside the skull base, orbit, optic nerve, or carotid artery
  • CSF rhinorrhea
  • Benign Sino-nasal neoplasms
  • Malignant Sino-nasal neoplasms
